11月16日，Vertex Pharmaceuticals和CRISPR Therapeutics共同宣布基因编辑疗法产品CASGEVY™（Exagaglogene autotemcel）获英国药品监管机构MHRA有条件批准上市，用于治疗治疗镰状细胞病（SCD）和输血依赖性β-地中海贫血（TDT），是全球首款获批上市的CRISPR基因编辑药物。

镰刀状细胞贫血病和β-地中海贫血症

镰刀状细胞贫血病和β-地中海贫血症（TDT）均属于一种遗传性血液疾病，其致病机理类似，都是β亚基突变导致的单基因遗传性疾病。胎儿血红蛋白（HbF）是携带氧气的血红蛋白，在胎儿发育期间天然存在，出生后转变为成人形式的血红蛋白。BCL11A是一种转录因子，可抑制红系细胞中的γ-珠蛋白和HbF表达。因此，靶向抑制BCL11A在理论上可以重新激活γ-珠蛋白表达，从而治疗地中海贫血症和镰状细胞病。

CASGEVY™疗法

CASGEVY™是一款自体、体外CRISPR/Cas9基因编辑疗法。旨在利用CRISPR/Cas9在体外编辑患者自身的造血干细胞，靶向BCL11A红细胞系特异性增强子（红系增强子）区域并且特异性沉默BCL11A基因，重启γ珠蛋白（与α珠蛋白组成胎儿血红蛋白HbF）的表达，从而治愈β地中海贫血。通过CASGEVY™治疗，可以提高HbF水平，有可能缓解输血依赖性β地中海贫血患者的输血需求，并减少镰刀型细胞贫血病患者的疼痛和使人衰弱的血管闭塞性危象。
